YOUR EMPLOYER is providing you with an affordable group comprehensive medical program designed to cover serious illness or injury. To do so, the plan contains deductibles, coinsurance and  copayments for which you are responsible. "GAP" Plan has been specifically created to help cover these out-of-pocket expenses.

As an Employee, It is Important you know the Facts!

·         Minimum Number of enrolled employees -5

·         No Medical Underwriting or Pre-Existing         

        Condition clause

·         Employees can elect to purchase the "GAP" Plan

        dependent coverage on a voluntary basis with no

        medical underwriting

·         The In-Patient Benefit amount ranges from $500   

        to $5000- available in increments of $500. The

        Out-Patient Benefit is 50% of the In-Patient

        Benefit
